Your Appointment

Please bring your current dental and medical insurance cards, surgical referral slip and any x-rays, and a list of current medications you are taking.

Your first appointment will consist of a consultation where you will receive diagnosis and treatment options. Your financial responsibility will also be outlined during your initial visit. All patients under 18 years old must be accompanied by a parent or guardian.

Please follow all pre-operative and post-operative instructions, as provided by Dr. Green and his staff. If you are unclear about these instructions, please call to clarify before your surgery day.

For your convenience, we accept cash, Visa, Mastercard and Discover. CareCredit financing is available to all patients needing a financing option. Dr. Green and his staff deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time of service.
